***Please include your home mailing address in your email. Certification packets are free of charge due to funding from Sheboygan & Plymouth Area United Way.

We also have WI State Licensing books available for purchase that apply to all counties we serve. These are published by the Department of Children and Families (DCF) and are a necessary and valuable resource when starting up a child care business.

• DCF 251 Group Licensing book $15.00

In additon we have the Family & Group Licensing Inquiry packets available for purchase (which include the corresponding Licensing Book).

• Family Licensing Inquiry Packet (includes book) $25.00
• Group Licensing Inquiry Packet (includes book) $25.00

Please send a check, payable to Family Connections, Inc., in the correct amount and specify if you would like a Family or Group, Licenings Book ($15.00) or Inquiry Packet ($25.00) includes the licensing book.
